Building on the success of our Chronic Care Management program, we’re proud to introduce remote patient monitoring, a breakthrough technology that can help you better manage your chronic conditions from the comfort of your home, while also helping us make better treatment recommendations.
Remote patient monitoring devices (blood pressure monitor, scale, glucometer, or pulse oximeter) independently transmit readings to your care team, providing valuable data for your physician to assess your overall health and risks of developing complications.
Once enrolled into the program, devices are delivered directly to your home. The only thing that you need to do is to take your readings as instructed, and we take care of the rest!

Remote Patient Monitoring is a program using technology that enables your care team to monitor and collect your important health readings such as blood sugar, blood pressure, and weight outside of a traditional healthcare environment, in other words in your own home or office. Using cellular (4G) devices, your readings are sent automatically with ease directly to our Care Managers for review. Through RPM, your Provider has access to robust, detailed, and often real-time physical data sets. This, in turn, gives a clearer and holistic view of your health and treatment progress and enables timely interventions and more informed care plans.

The data and medical information obtained from your participation in the program may/may not be considered part of your patient record, but may be shared with other health care professionals to enhance your care.

This program will allow you to take control of your high blood pressure with the support of your Care Team. Once you are enrolled in the pilot RPM program, you will be equipped with an FDA approved medical grade device such as a blood pressure monitor. Your FDA medical grade device is already programmed, ready for you to start monitoring your blood pressure!
