Compass Medical PC is working with Medicare to improve patient care by participating in an Accountable Care Organization, (ACO). We work hard to help our eligible patients gain more control of their health by staying connected to caregivers in between routine healthcare visits! CHRONIC CARE MANAGEMENT (CCM)

This program will allow eligible patients take control of their health with the support of your Care Team. We will work with you on your schedule and your priorities to provide the following:

  • Patient-Centered care plan
  • Telephone calls in between office visits to help manage your chronic health conditions
  • Consistent communication and involvement between you and your care team
  • Coordination of care with outside providers

REMOTE PATIENT MONITORING (RPM)

Remote Patient Monitoring allows eligible patients to receive devices to remotely collect important healthcare readings. Using cellular (4G) devices, your readings are sent automatically with ease directly to our Care Managers for review. This enables your care team to monitor and collect your important health readings such as blood sugar, blood pressure, and weight outside of a traditional healthcare environment, in other words in your own home or office. Through RPM, your Provider has access to robust, detailed, and often real-time physical data sets. This, in turn, gives a clearer and holistic view of your health and treatment progress and enables timely interventions and more informed care plans.

TRANSITIONAL CARE MANAGEMENT (TCM)

The transitional care management program is designed to facilitate a patient’s transition home from an inpatient stay in a hospital or skilled nursing setting A nurse will contact the patient to address any issues, review medications and schedule a follow-up visit with the primary care provider within 2 weeks of being discharged, assuring immediate access to care and avoiding potential re-hospitalization.

DIABETES EXCELLENCE PROGRAM

The Diabetes Excellence Program consists of a diverse team of endocrinologists, nurse practitioners, certified diabetes educators and registered dieticians/nurses who work to provide tools, strategies and resources to help prevent serious health complications due to diabetes to ensure a better and healthier lifestyle overall. Our team is dedicated to delivering comprehensive medical management, education, nutritional counseling and support to empower our eligible patients, their families and caregivers with the knowledge and skills to manage their diabetes properly.

ANNUAL WELLNESS VISITS

Along with a yearly physical exam, annual wellness visits play an important role in supplying eligible patients with a comprehensive care management plan. These visits allow healthcare providers to gather important patient information such as medical and family history, health risks, and specific vitals in order to update providers’ creation of personalized prevention plan containing tools, strategies and resources to living a healthier life.

EMERGENCY ROOM FOLLOW UP

Similar to the Transitional Care Management program, a clinical provider will contact patients after a visit to the emergency room to offer post-emergency care assistance and book follow up appointments with your primary care provider as necessary.
